If you've never visited Tranquility before, you'll need to download these forms and fill them out before your first visit. These forms will collect your personal information, past medical history, and insurance information.
Terms of Acceptance, Informed Consent & HIPAA
These forms must also be filled out prior to your first visit. By filling out these forms, you are confirming your knowledge of chiropractic and allowing Dr. Jenn to proceed with care. You are also acknowledging our standards in patient privacy through HIPAA standards.
